Abstract

Aluminum phosphide(ALP)is a cheap, effective and commonly used pesticide for protection of stored grains.However,it is now one of the most common causes of poisoning among agricultural pesticides. Human poisoning
with ALP is associated with very high mortality. Death rates are highly
variable, ranging from 37% -100% even in well prepared hospitals.The increased incidence of acute ALP poisoning and its high mortality
is a real challenge for health professional staff in developing countries. There is no specific antidote and the treatment is mainly supportive. Hence,evaluation of new treatment modalities is necessary, in order to decrease the rate of mortality and/or morbidity. Therefore, the aim of this study was to evaluate the efficacy and safety of intra venous lipid emulsion as an adjuvant therapy for acute aluminum phosphide poisoning.
This study was a randomized controlled trial that was conducted on 50 patients presented with acute ALP poisoning. The study participants were recruited from the Poison Control Unit of Tanta University Emergency Hospital, starting from December 2016 till November 2017. Fifty patients were randomly allocated into experimental and control groups (25 patients each) using the sequentially numbered, opaque sealed envelopes method. The experimental group received intravenous lipid emulsion 20% as IV infusion at a rate of 10 ml/h and gradually tapered in addition to the usual convention
treatment. The control group received only the conventional ALP treatment.The patients’ personal and medical data, clinical characteristics,investigations and treatment data were recorded in a special observation sheet for every
patient.
