Abstract

The thesis was based on two research studies– one directed towards the development of a process safety capability maturity model, framework (PSCMM) with high objectivity in evaluating the effectiveness of process safety KPIs implementation process during various steps of application. And the other directed towards providing a tool to find out the most cost-effective corrective actions after a major incident by improving the data utilization from earlier studies to address the direct and root causes of the event.
The first proposed framework was tested on data collected from the database of an oil and gas joint venture company that operates two offshore fields in the Suez Gulf in Egypt. The practical application of the proposed safety capability maturity framework process worked very well. The results uncovered the hidden hazards and gave deep insights into the effectiveness of the implementation of the safety process (KPIs).
The second research proposed a less time-consuming tool for root cause analysis.The tool was demonstrated and tested using the piper alpha tragedy as a case study. The proposed tool is not meant to substitute, but to supplement the existing methods used in root cause analysis. The most distinguished feature of the tool is that it identifies incidents contributors and the timing errors as well as gives ideas on their removal.
