Abstract

1-Introducation: Diabetes mellitus (DM) is a major rapidly growing public health care problem in all age groups. Aim of the work: Diabetes mellitus (DM) is a major rapidly growing public health care problem in all age groups. To investigate the pulmonary function abnormalities relationship to the duration and the level of control of T1DM. To evaluate the levels of adiponectin in T1DM children and adolescents, and to explore its possible relationship to the occurrence of pulmonary function abnormalities.3- Subjects and Methods: The study was carried out on 60 children and adolescents with T1DM (age ranges between 6-18 years) recruited consecutively from Diabetes Outpatient Clinic at Mansoura University Children’s hospital (MUCH) according to the following eligibility criteria. Fifteen age and sex matched healthy children and adolescents are involved as a control group. Results: Our study results showed significant negative correlation between T1DM duration and PFTs parameters, in addition there was significant negative correlation between PFTs parameters and level of Diabetes control evidenced by elevated levels of glycated hemoglobin. Adiponectin showed significant positive correlation with T1DM duration while negative correlation with PFTs parameters. Discussion: Individuals with affected PFTs were significantly older in age, had longer T1DM duration, had higher HbA1c, and higher serum adiponectin level. There was no significant difference between T1DM patients with affected PFTs and those with non-affected PFTs as regard age at onset of diabetes, sex, anthropometric measurements or lipogram. Summary and Conclusion: Longer duration of Diabetes and poor glycemic control are associated with increased risk of restrictive pattern of ventilatory dysfunction. Adiponectin can be used as a marker for early screening for pulmonary dysfunction. There is alarming increase in the incidence and prevalence of DM worldwide, but the incidence is rising rapidly in the Arab world. In childhood, the most common form of diabetes all over the world is type 1 diabetes (T1DM), an autoimmune disease with pancreatic β cells destruction and absolute lack of insulin. The current study enrolled 60 diabetic children who were subdivided into 4 groups based on duration of diabetes in addition to 15 age and sex matched controls. All individuals were subjected to full history taking, clinical examination and laboratory investigations in the form of spirometry, serum adiponectin level and HbA1C.
