Abstract

Background: The achievement of safe childbirth by cesarean section (C.S.) was one of the greatest medical advances of the 20th century. Indeed, C.S. is now the most frequent operation performed, constituting approximately one third of all the deliveries annually. Epidemiologic data report a C.S. incidence of 20%–30% worldwide, with comparable rates in high-income and low-income countries. Aim of the Work: The aim is to compare the effectiveness of carbetocin versus syntometrine in prevention of postpartum hemorrhage after cesarean section. Patients and Methods: Prospective double-blinded randomized controlled clinical trial. The study was conducted at Manshiet El-Bakry General Hospital at time between August 2017 till January 2018. 348 women for elective caesarian section divided into two groups each including 174 women. The 1st group received carbetocin, the other group received syntometrine. Informed consent was taken from each woman before participation in the study. Results: A total of 348 women, candidates for elective caesarian section, divided into two groups each including 174 women. The 1st group received 100 μg carbetocin (pabal), the other group received syntometrine (5 IU Oxytocin and 0.2mg ergometrine). After fetal extraction and before placental removal the drugs were administered through IV route. Analysis of the study results showed the absence of any statistically significant difference between the two groups as regards data including age, maternal BMI and gestational age. It also showed that there was no statistical difference between the two groups as regards parity and times of abortions. Also the difference between mean SBP and DBP between the two groups was statistically insignificant. Conclusion: The results of the thesis indicate that carbetocin is more effective than syntometrine for the prevention of postpartum hemorrhage and decreasing postoperative blood loss during caesarian section. Thesis also proved that drug related side effects (nausea/vomiting, Headache, and Chilling). are more likely to happen with carbetocin. Also, carbetocin was insignificantly associated with the need for additional uterotonic drugs.
