Abstract Within the last decade, the laparoscopic appendectomy has gained popularity. During laparoscopic appendectomy, any other pathology that comes to view could be diagnosed rather easier and dealt with in the same time. The decision as to which of these two techniques to employ is usually based on the surgeon’s preference rather than on knowledge of the advantage of each. Forty patients with suspected complicated acute appendicitis were admitted to ER in the Menofia University &Shebein El Kom Teaching Hospital .All were randomly submitted to open or laparoscopic appendectomies. They were randomized in two groups group A: including (20) patients for whom laparoscopic appendectomy was done group B: including (20) for whom open appendectomy was done. The study was conducted after receiving approval from the local ethics committee and informed consent from the patients to be included in the study. We conducted a prospective study comparing differences between patients undergoing laparoscopic and open appendectomy as regard: Operative time: The time from the incision time till closure of the port site or the wound. Intra-operative complication. Early post-operative complications. Drain insertion. Time of oral intake. Length of hospital stay, time from the surgery till the discharge calculated in days. The aim of this study was thus to ascertain whether differences exist between laparoscopic appendectomy and open appendectomy techniques so that an evidence-based decision could be made. A prospective study was conducted on forty patients who underwent either laparoscopic appendectomy procedure (20patients), or open appendectomy (20patients), all at Menoufiya University Hospital. A determination of which of the two techniques employed was chosen randomly. In laparoscopic appendectomy the monopolar diathermy was used to divide the mesoappendix and controlling the appendicular artery then the base was cut between two ligatures whether by endoloops or intracorporeal suture. Comparing laparoscopic appendectomy and open appendectomy resulted in significantly less surgical time for laparoscopic appendectomy. Also laparoscopic appendectomy has the advantage of producing less intraoperative and early post-operative complications time, shorter post-operative ileus, and shorter hospital stay. |
