Abstract

Oral lichen planus (OLP) is a common chronic inflammatory disease mediated by T cell affects the oral mucosa and skin. Although the etiology of OLP is unknown, several factors have been involved, including stress, hepatitis C, diabetes, trauma, genetic predisposition, and viral infections.OLP affects women more than men at a ratio of approximately 1.4:1
and occur predominantly in adults. The exact pathogenesis of the disease remains unclear, Activated T cells release cytokines leading to the attraction of inflammatory cells and the
destruction of keratinocyte by cell mediated cytotoxicity. One of this cytokines is TNFα that plays a pivotal role in innate inflammatory responses, and it has been implicated in the pathogenesis of several chronic inflammatory disorders with an autoimmune component, such as OLP. Recently it has been suggested that increased reactive oxygen species(ROS)and lipid peroxides may play a part in the pathogenesis of various diseases such OLP. There are several forms of OLP, the most common clinical form are
reticular and erosive forms. Erosive form of OLP can cause symptoms ranging from burning sensation to severe pain, interfering with speaking,eating and swallowing. Thus, symptomatic OLP patients often require therapy to reduce pain and burning sensation. Various treatments have been used to treat symptomatic OLP, but a complete cure has not yet been accomplished because of its recalcitrant nature.Topical steroids are widely used in the treatment of OLP to reduce pain and inflammation.
