Abstract

Chest infection is an infection of the lung parenchyma developing in patients on mechanical ventilation for more than 48 hours. It is a common problem in the Neuro-critical care unit, if they were inadequately managed; chest infection is associated with increased morbidity and mortality rates, in addition to increased cost and the period of hospital stay, so the nurses’ role is essential in the prevention and early detection of this common problem with patients in Neuro- critical care unit. The nurses’ role is a complex performance implies specific set of knowledge and practice needed for providing specific nursing measures.
Aim of the study:-
This study aimed to assess the nurses’ performance regarding management of chest infection in neuro-critical care unit through:
1. Assessment of nurses’ knowledge regarding management of patients with chest infection in neuro-critical care unit.
2. Assessment of nurses’ practice regarding management of patients with chest infection in neuro-critical care unit.
Research question:
To fulfill the aims of this study the following, research questions were formulated:
I. What was the nurses’ level of knowledge regarding management of patients with chest infection in neuro -critical care unit?
II. What was the nurses’ level of practice regarding management of patients with chest infection in neuro-critical care unit?
The present study was portrayed under the four main designs as following:-
I- Technical design.
II- Operational design.
III- Administration design.
IV- Statistical design.
2- Technical Design:
The technical design included; the settings, subjects, and tools for data collection used in the study.
Research design:
A descriptive exploratory design was conducted in this study.
Setting:
The study was conducted at the neuro-critical care units (neuro critical unit 1, new neuro –critical care unit and stroke unit) at Bab Elsheria Governmental Hospital (Alzher University).
Subject:
Convenience sample of 50 nurses (12 nurses from NCCU, 6 nurses from Stroke unit &32 nurses from the new ICU) who were caring for patients with chest Infection in the above mentioned study settings, which was included in this study. Studied nurses were females, with different ages, different educational levels and different years of experience.
Tools of data collection:
Two tools were used in the current study as the following:
(I) Self-administered questionnaire:-
This tool was divided into two parts as follows:-
Part (I):-
It was concerned with assessment of demographic characteristics of studied nurses.
Part II:-
This part was developed to assess nurses’ knowledge regarding chest infection and the nursing management for those patients.
(II) Nurses practices observational checklists:- Researcher based on reviewing the related literature. It was used to assess nurses’ practice related to management of patients with chest infection in neuro-critical care unit.
It included seven parts: -
I. I-Continuous vital signs monitoring.
II. Oxygen therapy (nasal cannula-mask oxygen).
III. Chest physiotherapy.
IV. Deep breathing exercises.
V. Range of motion
VI. Care of Patient on MV (Suctioning &ETT Care).
VII. Application of infection control measure.
Results:
The results of this study have shown the following:-
• Near half (48%) of the studied nurses their age less than 25 years with a mean age of 28.2± 6.45. Approximately half of them were having less than 5 years of experience with a mean of 7.4± 5.2.All of the nurses (100%) were females. Near half (46%) of them graduated from Secondary school of nursing. Two thirds (82%) of them had no previous training courses. More than half (52%) of them Work in Neuro-Critical Care Unit according to their wishes.
• In relation to total knowledge about disease, near half (52%) of the studied nurses had unsatisfactory level about total knowledge regarding management of chest infection.
• Near two thirds (62%) of studied nurses had unsatisfactory level regarding total practice of Oxygen therapy.
• More than half (58%) of the studied nurses had unsatisfactory level about measuring respiration ,and three quarter (76%) of them had unsatisfactory level regarding measuring temperature. In relation to total practice of vital signs more than half (58%) of them had unsatisfactory level.
• Two thirds (64%) of the studied nurses had unsatisfactory level regarding Oropharyngeal and nasopharyngeal suctioning practice.
• More than half (60%) of the studied nurses had unsatisfactory level regarding Chest physiotherapy exercise (CPT) practice and Deep breathing Exercise practice.
• Two thirds (66%) of the studied nurses had unsatisfactory level regarding total Practice about Application of Infection Control to Prevent Chest Infection
• Near than three thirds (86%) of studied nurses had unsatisfactory level regarding total practice about Range of Motion.
• More than half (58%) of studied nurses had unsatisfactory level regarding total practice about chest infection.
• Three quarter (72%) of the studied nurses had unsatisfactory level regarding assessment, planning and implementation regarding ROM exercise.
