Abstract

Glaucoma is a specific optic neuropathy causing characteristic visual field changes in which increased IOP is the main risky factor. Glaucoma is an important factor of blindness. Acute congestive glaucoma is an emergency condition in ophthalmic practice. The incidence of acute congestive glaucoma had been decreased due to early cataract surgery. Cataract surgery with IOL implantation is associated with anterior chamber changes, in the form of increase the angle width and anterior chamber depth as well as increase of aqueous drainage and hence decreases the IOP. It also prevents peripheral anterior synechae, which is responsible for chronic narrow angle glaucoma. This study assesses those patients who have acute or narrow angle glaucoma with significant cataract. Patients who have previous intra-ocular surgery, diabetic retinopathy, vascular occlusion (vein or artery), uveitis or secondary glaucoma had been excluded. The study assesses forty (40) eyes that have chronic or acute narrow angle glaucoma with significant cataract. All patients had been collected from out-patient clinic at Mansoura Ophthalmic Center. All patients had been informed with written consent about the study and understood the plan of management. After that, they had been examined ophthalmologically to discover signs of the problem. The examination included visual acuity assessement, detailed anterior and posterior segment assessment. Twenty (20) eyes had undergone phaco-emulsification with IOL implantation, and twenty (20) eyes had undergone phaco-emulsificatio and IOL implantation with sub-scleral trabeculectomy as a conventional manangement. The results are collected and statistically analyzed as well as published. We found that the phaco-emulsification with IOL implantation is associated with comparable results with the conventional surgery (phaco-emulsification with IOL implantation and sub-scleral trabeculectomy) but the later is associated with higher incidence of complication.
