Abstract

Otosclerosis is a focal or diffuse spongifying disease of the bony labyrinth. The otosclerotic focus may be asymptomatic, or may cause progressive conductive, mixed or occasionally sensorineural hearing loss. Otosclerosis is an important cause of auditory and, to a lesser extent, vestibular manifestation. Also less commonly encountered than hearing loss, vestibular symptoms and signs in otosclerosis where more common than generally appreciate. Videonystagmography (VNG) is of great value in evaluating vestibular function in these patients. Accordingly, this study was done to evaluate the vestibular system function in otosclerotic subjects before and after stapedectomy using VNG. This study included thirty subjects with clinical bilateral otosclerosis. Basic audiological evaluation in the form of pure tone audiometry, speech audiometry, tympanometric testing and VNG was done to these patients preoperatively and one month after stapedectomy. The result of this study showed improvement of the hearing threshold after stapedectomy. VNG abnormalities in the form of unilateral caloric weakness before stapedectomy where present in 2 patients and after stapedectomy we recorded unilateral caloric weakness in 7 patients. It was recommended to assess the vestibular system function in otosclerotic subjects before and after stapedectomy as vestibular manifestations in otosclerosis are more common than generally appreciated.
