Abstract

Sciatic nerve palsy is a well-described complication following total hip arthroplasty, but is highly distressing and disabling. A sciatic nerve palsy may cause difficulty with the post-operative rehabilitation, and overall mobility of the patient. Sciatic nerve palsy may result from compression and tension to the affected nerve(s) during the course of the operation via surgical manipulation and retractor placement, tension from limb lengthening or compression from post-operative hematoma. In the literature, hip dysplasia, lengthening of the leg, the use of an uncemented femoral component, and female gender are associated with a greater risk of sciatic nerve palsy. Total hip arthroplasty (THA) is one of the most clinically successful and cost-effective medical procedures developed during the last century.This study was targeting patients with sciatic nerve palsy after primary total hip arthroplasty admitted at Mansoura university hospitals, arthroplasty unit of orthopaedic department from January 2012 to January 2015, and their follow up which was planned for one year at outpatient clinic of arthroplasty unit, Mansoura university orthopaedic center.The study included 72 male (62.1%), and 44 female (37.9%), the age of the patients ranged from 17 to 80 years with a mean age of 43years. The study included 15 cases (14.9%) with both sides operated and 86 cases with only one side done (85.1%). The study included 113 hips (97.4%) with cementless total hip replacement, and three hips (2.6%) with cemented THR.Sciatic nerve injury reported in (5.2%) of 116 hips. Most of patients with injured sciatic nerve were younger than 60 years (4.85 %), while only one hip (7.7%) is older. It is found in (5.6%) of men and (4.5 %) of women. In the study; (4.7%) with sciatic nerve injury in which one side was operated and (6.7 %) with sciatic nerve injury in which both sides was operated.The study included six cases with Sciatic nerve injury following primary total hip replacement five cases (83.3%) had recovered, and one case did not recover. Conclusion: Sciatic nerve palsy post primary total hip arthroplasty is a highly distressing and disabling complication. Prognosis of sciatic nerve palsy after primary total hip arthroplasty is generally good. Sciatic nerve injury reported in (5.2%) of 116 hips. Most of patients with injured sciatic nerve were younger than 60 years (4.85 %), while only one hip (7.7%) is older. It is found in (5.6%) of men and (4.5 %) of women. In the study; (4.7%) with sciatic nerve injury in which one side was operated and (6.7 %) with sciatic nerve injury in which both sides was operated.
