Abstract

Background: Decrease of liver function in chronic liver diseases can lead to some degree of malnutrition. The degree of malnutrition increases with deterioration of the liver function. Nutritional status of the patients should be evaluated starting with a nutritional screening which is followed by a full assessment of at risk patients. Objectives: to assess the nutritional status of patients with chronic liver disease attending the gastroenterology and hepatology department of Ain Shams university hospital, identify the relationship between hepatic disease severity and degree of malnutrition and to measure the degree of patient’s compliance with the current international nutritional guidelines. Methods: A cross sectional study was carried out on a sample of 161 chronic hepatic patients in gastroenterology and hepatology department of Ain Shams university hospital. An interview questionnaire containing sociodemographic data, special habits of medical importance, past history of medical diseases and gastrointestinal symptoms was used. Then anthropometric and biochemical assessment were carried out. Liver disease severity was assessed by using Child Pugh score. The collected data was processed and coded before being analyzed using SPSS program version 22. Results: The mean age of the studied patients was 51.3 years. Males represented 51.6% of the patients. About 42% were classified as Child B and only 11 % were graded as Child C. About 50.9% of the studied patients were moderately malnourished (PG-SGA B), while 16.8% were severely malnourished (PG-SGA C). Generally, results revealed that prevalence of malnutrition ranged from 87.6% (by RFH-SGA) to 12.4% (by BMI) according to the different methods of assessment. There was a statistically significant difference between the three Child groups as regards different nutritional assessment parameters. Conclusion & recommendation: There was a high prevalence of malnutrition in chronic hepatic patients in Ain Shams University hospital. The percentage of malnutrition increases with the increase in the severity of liver disease. Accordingly, proper nutritional screening and assessment should be done routinely to chronic hepatic patients
