Abstract

The study was conducted on nine patients attending the outpatient clinic of plastic surgery department at Assiut University Hospital and El-Helmia Military Hospital between June, 2014 and November, 2015 with long standing radial nerve injury, long standing low median nerve injury and failed repair of flexor pollicis longus or extensor pollicis longus tendons.
In this study we used Flexor digitorum superficialis muscle tendon transfer for restoration of thumb function in 3 groups:
group A: Restoration of thumb flexion in 3 cases.
group B: Restoration of thumb extension in 3 cases.
group C: Restoration of thumb opposition in 3 cases.
The aim of this study is to evaluate the role of flexor digitorum superficalis tendon transfer as a work horse for restoration of thumb functions (flexion, extension and opposition) after failed repair or negligence of nerve and tendon injuries.
Evaluation measures included
1- Subjective self evaluation of results by the patient.
2- Objective assessment of restored thumb movement.
We concluded that flexor digitorum superficialis tendon transfer is a relatively easy procedure to restore thumb functions that has distinct advantages compared with other options to restore thumb function and provides very good results.
One of the distinct advantages of Flexor digitorum superficialis tendon transfer is that it is a suitable option for restoration of thumb functions compared with other options as the FDS has a very good excursion, easily harvesting and minimal donor site morbidity.
Flexor digitorum superficialis tendon transfer is a very good option or may be the first option in restoring thumb opposition in low median nerve injury, restoring thumb flexion and it’s a very accepted option in restoring thumb extension.
Flexor digitorum superficialis tendon transfer doesn’t affect wrist flexion and it’s important to maintain wrist flexors in the patients who earn their living from being workers.
The main disadvantage of flexor digitorum superficialis tendon transfer is the weakness of the grip however that weakness is insignificant functionally.
Flexor digitorum superficialis tendon transfer is considerd a work horse for restoration of thumb functions. Careful preoperative planning and aggressive postoperative rehabilitation are critical to optimize results. When this is done, full range or near full range of flexion and extension of thump IP joint or up to full palmar abduction could be achieved.
