Abstract

Irritable bowel syndrome (IBS) is a highly prevalent disorder. It is found In 10% to 20% of individuals using standard diagnostic tools such as the Rome II criteria .The recent development of highly sensitive and specific serologic assays for Celiac disease has led to the increased realization that the disease is more common than it was believed . This justifies the concern that some IBS-labeled patients may in fact have Celiac disease.
The aim of the present work is to estimate the prevalence of Celiac disease in patients who as IBS, and also to describe their clinical features. Eighty patients with IBS diagnosed by application of the Rome II criteria collected from our outpatient internal medicine clinic .Serum samples were taken and testing for anti-tissue transglutaminase (anti-tTG) serology was performed using the Anti-Tissue-Transglutaminase IgA and IgG in one test called tissue transglutaminase screen by automated ELIZA machine called alegria. Then we did uper endoscopy for the patients who are postive for anti-tTG and doudenal biopsy was taken for histopathological examination.
Two patients tested positive for anti-tTG antibodies, and duodenal biopsies confirmed the diagnosis in them. Thus, in this patient population with presumed IBS, 2.5% actually had CD.
CD is common in patients with presumed IBS. Because the manifestations of CD respond to a gluten-free diet, testing for CD in patients with presumed IBS may prevent prolonged morbidity and unnecessary health care costs.SO routine screening for CD in patients with symptoms of IBS is recommended.
