Abstract

Summary and Conclusion
- Gray scale sonography (GSS), Color Doppler sonography (CDS) and Spectral Doppler (SPD) are three sonographic methods used for the evaluation of different SG lesions and they are relatively inexpensive, non-invasive, valuable and useful methods for differential diagnosis of benign and malignant SG tumors.
-The aim of our study was to evaluate the accuracy of GSS, CDS and SPD in the differentiation between benign and malignant SG tumors.
-We examined 44 patients complaining of benign and malignant SG tumors. 28 of them were histopathologically proved to be benign and 16 of them were found to be malignant and we compared them with the contra lateral normal SG.
-We found that the normal salivary gland parenchyma had minimal blood flow that could not be detected by CDS and SPD.
-By using GSS, all malignant and benign SG tumors were hypoechoic. Malignant SG tumors showed significantly higher incidence of ill defined borders and lymph nodes involvement than benign tumors. While, other sonographic criteria (homogenicity and shape) were not significantly different.
-By using CDS, malignant SG tumors showed significantly higher degree of tumor vascularization with scattered distribution.
-By using SPD, malignant SG tumors had significantly higher values of PSV, RI and PI than benign tumors.
-We found that the RI and PI were the most accurate indices followed by the PSV, then the borders and lymph nodes involvement and finally vascularity grade and distribution in differentiating benign from malignant SG tumors.
-Finally SPD can exclude malignancy when RI <0.76 and PI <1.22.
