Abstract

Senile cataract is the most common vision related problem affecting older adults. In Egypt, cataract is responsible for 60% of blindness. Surgical cataract extraction remains the only effective treatment available to restore or maintain vision. Cataract surgery is a unique experience for each patient. It might be a stressful big challenge for the elderly as it is associated with a high level of anxiety that will arise at least one week before cataract surgery. Anxiety before cataract surgery has a devastating effect on its outcome. Consequently, there has been a growing interest in the study of non-pharmacological anxiety reducing nursing interventions. To date, the researcher did not elicit any research study based on a nursing model designed to be used by nurses in order to decrease the level of perioperative anxiety of geriatric patients undergoing cataract surgery. Therefore, this study aims to examine the effect of implementing anxiety reducing nursing interventions based on Peplau’s Interpersonal Relations Model on perioperative anxiety among geriatric patients undergoing cataract surgery.
Materials and Method
Research design: The study used a quasi-experimental design.
Setting: The study was carried out in the surgical inpatient department of the Alexandria ophthalmology general hospital (Farouk).
Subjects: The study subjects comprised fifty (50) geriatric patients aged 60 years and above, diagnosed with senile cataract, scheduled for elective cataract surgery for the first time, under local anesthesia. The study subjects were divided into two equal groups twenty five (25) each.
• Study group: - Received the anxiety reducing nursing interventions based on Peplau’s Interpersonal Relations Model.
• Control group: Received the routine care of the inpatient department at the Alexandria Ophthalmology General Hospital Farouk).
Tools: Data was collected using the following tools:-
 Tool I: Visual Analog Scale for anxiety (VAS), it was used to measure the patient’s state anxiety
 Tool (II): Mini – Mental State Examination (MMSE), it was used to exclude patients with severe degree of cognitive impairment.
 Tool III: Geriatric Depression Scale- Short Form (GDS-SF), it was used to exclude patients with depression.
 Tool IV: Socio - demographic and clinical data structured interview schedule, it was developed by the researcher to collect information about socio-demographic characteristics of the study subjects and their medical health history.
 Tool V: The Amsterdam Preoperative Anxiety and Information Scale (APAIS), it was used preoperatively to screen the sources of study subjects’ anxiety and their information requirements in order to identify those in need for extra support.
 Tool VI: Geriatric patients’ knowledge and expectations structured interview schedule, it was developed by the researcher to determine the geriatric patients’ knowledge and expectations about cataract and its surgery.
 Tool VII: Physician rating scale for patient’s intraoperative compliance, it was used to assess the physician’s opinion about the patient’s intraoperative compliance during cataract surgery.
