Abstract ABSTRACT: Objective: To compare between vaginal hysterectomy and laparoscopic assisted vaginal hysterectomy in cases of large uteri regarding operative and postoperative outcomes. Study design: Prospective randomized controlled trial. Study setting: Ain Shams University Maternity Hospital. Patients: 50 patients were recruited in this study and they were divided into two equal groups, first group underwent vaginal hysterectomy and the second underwent laparoscopic assisted vaginal hysterectomy Results: Patient characteristics were similar in both groups. The mean (SD) postoperative pain score after 24hrs was significantly lesser in the LAVH group (VH: 1.4{50} vs LAVH: 1.13{0.33}, p value =0.03) ,. As for surgical outcomes, estimated intraoperative blood loss (P=0.90), operative time (P=0.48), Pre-operative hemoglobin (P=0.09), Postoperative hemoglobin (P=0.42), operative complications (P=1.0) didn’t differ between the two groups. The hospital costs was significantly higher in case of LAVH group (VH: 908.32EP{153.83} vs LAVH: 1330.40 EP{191.90}, P value >0.001). No significant difference exists in the duration of postoperative hospital stay between the two groups(VH: 49.92 hours{28.50} vs LAVH : 58.56{27.78}, P=0.28) nor the actual uterine measured postoperatively(VH: 350.72gm{71.78} vs LAVH: 385.96gm{172.52}, p=0.35). Conclusion: Both vaginal hysterectomy and laparoscopic assisted vaginal hysterectomy are safe procedures in cases of large uteri with no significant difference between them except in terms of costs as VH appears to be more cost effective. |
