Abstract

The introduction of hysteroscopy in gynecologic practice revolutionized the diagnosis and treatment of intrauterine disease. New methodological and technological developments have made diagnostic and operative hysteroscopy much more efficient, cost effective, safe, and useful. The most common indication for hysteroscopy is abnormal uterine bleeding (AUB), but it is also used in cases of infertility and Mullerian anomalies.
In our retrospective study we could follow up 272 from 762 patients who had operative hysteroscopy at (Early Cancer Detection Unit) at Ain Shams Maternity Hospital of Ain shams University over 6 years (2007-2013), (159 hysteroscopic adhesiolysis, 65 hysteroscopic myomectomy and 48 hysteroscopic septoplasty).
The highest mean age was encountered among patients of the study (43 years) and the least mean age (18 years).
As regards type of hysteroscopy used in the study population, resectoscope was done for 6 patients (0.8 %) and vaginoscope was done for 756 patients (99.2 %).
Only one patient (0.8 %) of our study had uterine perforation, this patient was suffering from uterine septum.
Infertility was the most reported complaint among all studies patients 84.4% and particularly higher among patients who had hysteroscopic adhesiolysis 58.5%, and repeated miscarriage reported as complaint among patients of study group in 15.6%.
Patients who had hysteroscopic myomectomy presented with fibroid had hysteroscopic myomectomy without intra or postoperative complication , most of cases in our study (96%) were done under general anesthesia for patient compliance , in the patient complaining of menorrhagia, condition was resolved after hysteroscopy, and recurrence rate of abnormal uterine bleeding was 10%. Patients who had hysteroscopic myomectomy for treatment of infertility, 66.2% of them got pregnant (43 out of 65 patients) (9 abortion, 5 preterm and 29 full term).
Infertility cases responded better for treatment among patients who had hysteroscopic septum resection 70.8% got pregnant compared to patients who had hysteroscopic adhesiolysis 32.1%.
Among patients who had done hysteroscopic septum resection 70.8% of infertile women got pregnant (34 out of 48 patients) (10 abortion, 5 preterm and 19 full term), the incidence of complication was 0.1% uterine perforation (one case from 48).
For patients who had hysteroscopic adhesiolysis, (51 out of 159 got pregnant) 32.1 %, (8 preterm, 5 miscarriage and 38 full term.
