Abstract

Evidence based practice in nursing (EBP) is a framework for clinical practice that integrates the best available scientific evidence with nurse’s expertise and the patient’s preferences and values to decide about health care of individual patients. In spite of all the various programs and strategies to promote the use of research findings, there is still a gap between theory and practice. In addition ,it is important to the health care professional for many reasons. At the top of this list, is the contribution of evidence to the effective care of patients. Studies have supported the conclusion that patient outcomes are substantially improved when health care is based on evidence from well-designed studies versus tradition or clinical expertise alone.(Leufer& Cleary, 2012). The EBNP for the standardized care of women affected by preeclampsia and eclampsia is guided by the best available evidence. The recent practice indicates that, current managements of preeclampsia and eclampsia are a reflection of past treatments a more current review of evidence based information on the management of preeclampsia. Further demonstrates that the mainstay of treatment has remained consistent. (Norwitz&Repke, 2009). The study was conducted at high risk unite at four hospitals which were namely (Mansoura University Hospital, Mansoura General Hospital, Mansoura International Hospital and Insurance Hospital)with in six months during the period from September 2014 to the end of February 2015 and data were collected by the researcher through two tools. Tool I: interviewing questionnaire sheet: This questionnaire was designed by the researcher and was based on reviewing literatures and reviewed by the supervisors. It was done in arabic form and consisted of multiple choice questions. It entails two parts as the following: Part 1: General characteristics of nurses as (Age, Marital status, Education level, Years of experience, Training programs……….). Part 2: Assessment of nurses’ knowledge about EBP of preeclampsia and eclampsia in pregnancy. Tool II Observational checklist: It was developed according to Royal College of obestetricians and gynecologist and WHO guidline for management of preeclampsia and eclampsia and reviewed by supervisors.It consisted of two parts . Part I:Observational ckecklist to evaluate nursing practice regarding evidence based in preeclampsia management. Part II:Observational ckecklist to evaluate nursing practice regarding evidence based in eclampsia management, Main results of the present study were as: That there was a highly significant difference between nurses’ general characteristics (age, education, employment, experience years and the working hours) and preeclampsia and eclampsia observational practice scores (P>0.05). About three quarters of nurses giving incorrect answer regarding the definition of evidence based nursing. Near to half of the sample had correct answer regarding the definition of eclampsia. The highest score for Knowledge level was (28.33±6.9) and practices of EBP regarding preeclampsia was (35.25±11.7) and eclampsia (43.9 ±14.15) management were among young nurses who represent the majority of the study sample. There was a significance difference between general knowledge of EBP and knowledge about EBP used in preeclampsia, but there was no significant difference between general knowledge of EBP and knowledge about EBP used in eclampsia. There were no practices done at excellent level regarding the recommendation of WHO for prevention and treatment of preeclampsia and eclampsia. There was no statistically significant difference between places of work, knowledge and practices of evidence based regarding preeclampsia and eclampsia management. There was no significant difference between different hospitals regarding general knowledge of evidence based Practice. Insurance hospital shows the highest mean score of knowledge (29.8 ±10.1) and observational practices of preeclampsia (32.0 ±21.4 (and eclampsia (36.7± 22.8), While general hospital shows the lowest mean score of knowledge (23.3± 9.0) observational practices of preeclampsia (20.2 ±12.1) and eclampsia (22.7± 13.9). Based on the results of the present study, the following are recommended: Improving nurse’s awareness regarding EBP through involving them in educational programs to encourage them to utilized EBP into patient daily care. Design and disseminate the guidelines to all maternity and MCH health care services to overcome this lack in nurse’s knowledge regarding EBP. Design and desseminate the guidelines to under graduate curriculum students nurse to be further applied into their practice after graduation. Training nursing staff on how to apply EBP into actual clinical care. Involving nurses into working groups about how use and apply EBP in patient care. High risk unit administrator help in improving nurses work through: Provide computer system for nurses to make patient record instead of using pen and paper to document. Encourage nurses on making researches and provide her the emotional and financial support that she needs. Further study is needed to investigate the reapplication of the present study on another setting and on large sample size.
