Abstract This study was conducted on 54 patients with acute Infection and feverish, during the first 24 hours of presentation.they were 37 females and 17 males, their ages ranging between 1-80 years with mean age 27.8 ± 21.8 years. They were recruited from the inpatients of Abbassia Fever hospital during(2012-2014). All patiens were subjected to full history taking, thorough clinical Examination besides: complete blood picture, ESR, liver and renal function, uric acid, iron study, total iron binding capacity, serum ferritin and hepcidin level in serum by Eliza technique. The control group were subjected only to CBC, ESR, chemistry, iron study, TIBC and serum level of hepcidin. The study revealed that: The mean serum hepcidin level was significantly higher in all studied patients compared to controls. The mean HB level was significantly lower in all studied patients compared to controls. The mean serum TIBC level is significantly lower in all studied patients compared to controls. The mean serum iron level is significantly lower in all studied patients compared to controls. In all studied patients the following correlations were found: The mean serum hepcidin level had significant positive correlations with age and significant negative correlations with the mean serum ferritin, total iron binding capacity, iron, total leucocytic count, mean cell hemoglobin, mean cell volume and significant negative correlation withthe mean hemoglobin level. finally, we can conclude that, in patient with acute infection there is a rapide increase in the serum hepcidin level which in turn decreases serum iron level and leads to anemia.
