Abstract

Pilonidal sinus is a disabling disease in young adults, which can result in an abscess or draining
sinus tracts, leading to long term loss of productive capacity and tends to have high morbidity. It
probably results from hair penetration beneath the skin.
Despite surgical therapy dating back to more than a century, management of this condition remains
controversial. Many different approaches ranging from non surgical treatment to extensive surgical
excision have been put forth but the method of simple lying open of the pilonidal sinuses is
considered as the gold standard.
There are many surgical approaches for the treatment of pilonidal sinus for example, excision and
healing by open granulation, marsupialization, excision and primary closure, creation of different
types of skin flaps, Radiofrequency and procedures using Nd-YAG or Ruby laser.
It was found that there were no difference in terms of wound infection or recurrence rates between
excision and primary closure, Marsupialization and skin flaps.
As regard morbidity, speed of healing of the wound and recurrence rates, karydakis and Bascom’s
procedures are being now the most suitable methods of treatment of pilonidal sinus.
As regard recurrent cases of pilonidal smus different types of skin flaps, Karydakis and Bascom’s
procedures are now the most reliable techniques for the treatment of recurrent cases.
The ideal treatment for pilonidal smus 1s one which aims at reducing hospital stay, minimizing
tissue destruction by avoiding unduly extensive procedures and the resultant complications, early
resumption of work by the patient while preventing recurrence.
