Abstract

The goal of the present study was to improve the quality of life among patients with functional dyspepsia .This was achieved through assessment of the prevalence of FD among primary care attendant, assessment of the prevalence of helicobacter pylori as a cause of FD among the studied group and study the impact of functional dyspepsia on the quality of life among the studied group before and after intervention.
The study was conducted in Munshaat Soultan Family health centre. Among 850 patients aged >18 to <55 years attended the clinic during the period 1st February 2013 to 1st December 2013, 318 patients fulfilled ROME III diagnostic criteria for FD and were enrolled in this study. Patients with FD were basically assessed using QOLRAD questionnaire and helicobacter pylori stool antigen testing. Hp +ve patients received helicobacter pylori eradication for one week , both Hp +ve and Hp-ve patients completed four weeks course of pantoprazole 40mg once daily, domperidone 10 mg premeal tid and life style modification, this was followed by on demand pantoprazole 40mg ,domeridone 10mg life style modification for six months. QOL reassessed at six weeks and six months while Hp stool antigen repeated at six weeks.
This study shows that the prevalence of FD is 37.4% of the studied patients, PDS is the most common subtype of functional dyspepsia (46.2%), and helicobacter pylori infection is positive in 41.8%.
There is statistical significant difference between helicobacter pylori positive (Hp+ve) and negative (Hp–ve) as regarding sex (p value <0.05) and different subtypes of functional dyspepsia.
The mean score of quality of life in patients with reflux and functional dyspepsia (QOLRAD) and its isolated domains are significantly lower among Hp +ve than Hp -ve patients (p value <0.001).
The dyspeptic symptoms improved in 84% of patients after PPIs and prokinetics, beside Hp eradication for Hp +ve. While not improved in 16% of patients and it is highly statistically significant.
This study shows that there is statistically significant improvement in QOL and its five domains (emotional distress, sleep disturbance, vitality, food/drink problems and physical social functioning) after six weeks treatment and after six months of on demand maintenance therapy in both Hp +ve and Hp –ve FD patients and also in isolated subtypes of FD (p value <0.001) .The effect size was large after short-term therapy and during maintenance treatment in both Hp +ve and Hp –ve patients. In Hp +ve patients there is statistical insignificant difference in QOLRAD and its isolated domains (sleep disturbance, Food/drink problems, Physical / social functioning) between six weeks and six months after intervention (p value >0.05). While vitality domain is statistically significantly lower six months than six weeks after intervention and (p value <0.05). In Hp –ve patients QOLRAD and its isolated domains is statistically significantly lower six months than six weeks after intervention and (p value <0.05)
