Abstract The uinary bladder serves as a reservoir for the storage and periodic release of urine. urine release is dependent on voluntary neural mechanisms that involve the brain and spinal cord. Continence is dependent on the ability of the detrusor to accommodate filling without significant increase in intravesical pressure, and on passive maintenance of sufficient urethral resistance to prevent leak of urine. In the male anatomy, there appears to be two functional urethral sphincters: a pmximai urethral sphincter, and a distal urethral sphincter. Any one of these two sphincters can maintain continence adequetly, and derangement of both of them produce incontinence. The study included 41 patients with derangement of their urethral sphincteric mechanisms. The etiology of derangement was congenital epispadias in four, rupture urethra secondary to pelvic fracture in 14, and prostatedomy either transurethral or retropubic in 23 patients. |
