Abstract

Comminuted fractures of tibia are one of the most challenging injuries in orthopedic surgery. Several methods have been described for the treatment of these fractures, including casting, external fixation,intramedullary nail and plate fixation. The choice of technique depend on multiple factors including fracture configuration, proximity to the joints, bone quality, soft-tissue injury,general condition of the patient and available equipment. Non-operative management may be appropriate in some patients, particularly those with contraindications to anaesthesia or surgery.The usual methods of treatment in comminuted fractures are conventional plate fixation, biological plate fixation, intramedullary nailing, pinning and plaster, and external fixators. Intramedullary nailing for tibial shaft fractures has been considered the standard of care with satisfactory results in most cases. External fixation offers benefits in terms of soft-tissue management and in the management of severely comminuted fractures, but questions remain about the risks of pin tract infection, nearby joint stiffness and patient’s inconvenience due to the bulky devices.
