Abstract

Female infertility has wide range of causes including: ovulatory disorders (ie.pituitary adenoma and polycystic ovarian syndrome), disorders of the fallopian tubes (ie.hydrosalpinx and pelvic inflammatory disease), uterine disorders (ie.mullarian duct anomaly, adenomyosis, and leiomyoma), and pelvic endometriosis.
The results of the MRI examination are used by the clinician in a comprehensive clinical assessment scheme to reach the final clinical diagnosis because of its multiplanar capability and good soft tissue contrast enhancement enabling accurate diagnosis of the lesions.
MRI should be done for all cases of infertility with clinical and /or laboratory abnormalities indicating hypothalamic pituitary axis involvement (i.e pituitary adenoma); indications may include neurological complaints or hormonal abnormalities such as elevated prolactine level.
The diagnosis and treatment of infertility results from the hypothalamic pituitary axis disorders depend on the results of cranial MRI on which the choice of treatment plane wither conservative or surgical done.
Although Laparoscopy, hysteroscopy, hysterosalpingography and ultrasonography are the most popular used techniques to evaluate female pelvic disorders related to infertility, magnetic resonance (MR) imaging has also been used to evaluate problems associated with female infertility. For example, MR imaging is well known to provide accurate information for differentiation of congenital uterine anomalies and detection and localization of uterine leiomyomas.
MR imaging also helps predict the outcome of conservative treatment for adenomyosis, leiomyoma, and endometriosis and may help patients select better treatment plans and management. MR imaging may also serve as an adjunct to diagnostic laparoscopy and hysterosalpingography for patients with hydrosalpinx, peritubal adhesions or pelvic adhesions related to endometriosis.
In conclusions,
MR imaging is a useful modality as an adjunct for routine infertility work-ups. Although the sonography remains the best screening procedure for the evaluation of a suspected pelvic lesion, the precise localization and tissue differentiation are better achieved with MRI. MRI is now considered the imaging modality of choice for diagnosis of hypothalamic pituitary lesions because of its multiplanar capability and good soft tissue contrast enhancement enabling identification of small lesion.
