Abstract

PE is the most common sexual problem encountered in male. It is of four types; primary (lifelong), secondary (acquired), natural variable and premature-like ejaculatory dysfunction.
The exact etiopathology is not known up till now. Genetic predisposition and disturbances in serotonin and its receptors involved in ejaculation (5-HT 1A & 2C) are supposed for the primary type. Chronic prostatitis, neuropathy and disseminated sclerosis are the suspected causes for the secondary type. Sexual abstinence is believed to be the cause of the natural variable PE. Over estimation and psychic disorders are believed to be the causes for the premature-like ejaculatory dysfunction.
There were no objective parameters to measure primary PE until Waldinger in 1994 introduced the IELT as an objective parameter that measure the time from vaginal penetration to ejaculation.
Primary PE definition was controversial until lately in 2007 when the ISSM defined it as; ejaculation always or nearly always within one minute of IELT and this occurs always or nearly always in all sexual encounters and causing distress to one or both partners.
Leptin is a fat hormone that regulates appetite, body weight and energy expenditure. This hormone received much attention in last years that discovered many other physiological functions on many body systems and organs. Unfortunately, these studies in the field of reproduction and sexual functions are very scarce. The few studies that exist in the literature point to the fact that it may - as serotonin - or through interacting with serotonin have a role in primary PE, but what is this role? The answer is unknown.
