Abstract

The term intestinal polyp is used to describe any projection arising from flat mucosa into the intestinal lumen. Polyps can be broadly classified as neoplastic and non-neoplastic.
Neoplastic polyps can be further classified as either adenomatous (premalignant) or malignant . Adenomatous polyps are common in industrialized countries, which suggest that dietary and environmental factors play a role in their development.
Non–neoplastic polyps classified into hyperplastic polyps, hamartomatous polyps and inflammatory . Hyperplastic polyps also have been described as metaplastic polyps. The hamartomatous polyp is constituted of the normal tissue found in the colonic mucosa with differences in proportion, distribution and overall composition. Inflammatory polyps are non-neoplastic proliferations of either mucosa or granulation tissue may be due to various injuries of the colorectal epithelium.
The aim of this work is a hospital basesd registration of all cases of colorectal polyps presenting to Ain Shams Hospitals from 2001-2005, in order to confirm the histopathological diagnosis and to detect the relative frequency of the malignant polyps to all other malignant tumors.
The cases of the study were retrieved from the Pathology labs of Ain Shams University Hospitals and Ain Shams specialized Hospital from 2001-2005.
Revision of the slides for histopatholgical evaluation was done, also the available clinical data as regard age, sex of the patient and site ,size of the polyps were collected.
The study included162 cases of colorectal polyps, 98 were non-neoplastic polyps and 64 were neoplastic polyps.
The results of the study showed that the malignant polyps represent 0.2% of all malignant tumors. As regard the age of the patients of non-neoplastic polyps, it ranged from 2 to 75 years, where cases of juvenile polyps showing the lower age group than the other non-neoplastic polyps. As regard neoplastic polyps the age of the patients ranged from 21 to 92 years.
Male cases were more predominant in both non-neoplastic polyps and neoplastic polyps, where male to female ratio was 1.6:1 and 1.2:1 respectively .
As regard adenomatous polyps the tubular adenoma is the most common type, which represented 50.7% of the neoplastic polyps. Juvenile polyp was the most common type in non-neoplastic polyps.
High grade dysplasia and colorectal adenocarcinoma were commonly seen in polyps with villous pattern where they represent 27 % and 45 % respectively.
Follow up is recommended for patients with previous history of colorectal polyp and for those who are first degree relative with colon cancer or adenomatous polyps.
