Abstract

prostatic specific antigen (PAA)is a serum maker for prostatic cancer. from 1998 to the present (PSA)has a main role in the diagnosis has resulted in a huge stage prostatic tumor.becouse sa is not prostate cancer-specific and prostate cancer develops in old men where the prevalence of benign prostatic hyperplasia is high several parameters have beendevelopedand investigated to enhance the sensitivity and specificity of the psa test these include psadensity serm psa concentration divided by the size of the prostate glands psa velocity(change in srum psa over time) age –specific reference ranges for serum psa and percent free psa. All four parameters have their criticism and are used to varying degrees by clinicians to improve the clinical utility of psa with regard to identify men with early curable prostate cancer from individuals with only benign prostatic hyperplasia.precent free psa may prove useful for staging prostatic cancer however further clinical trials are needed to determine its clinical utility psa is the most clinically useful mean to monitor men for disease recurrence following treatment of prostate cancer it is now possible to increase the lead time to disease recurrence by several monthes with ultrasensitive psa assays and reverse transcription polymerase chain reaction (RT-PCR)technology.many advances have occurred during the last decade in the clinical use of prostate specifice antigen for detection staging and monitoring therapy in men with prostate cancer much of the focus has been on improving the ability of this tumor marker to selectively detect prostate tissue using ultrasenesitive assays or RT-PCR for partical purposes psa remains prostate specific and is cuurently the best and most widly used tumor maker in urology today
