Abstract

200 cases of routine tonsillectomy in Benha university hospital and Benha teaching hospital (in Kalyobia Governorat) have been searched for foci of tuberculosis; no special selection of cases was done.
1-There was one case (0.5%) with positive history of unpasteurized milk intake during infancy and childhood in urban areas and there were 13 cases (6.5%) with positive history in rural areas.
2-there are thirty cases (15%) with positive history of contact with TB patients.
3-Enlargement of the deep cervical glands was noticed in about 90% of the patients, the nodes were solitary, Multiple and bilateral in all patients
4- 15 patients (7.5 %) show increase of ESR above 10 mm/h, the rest of them have the erythrocyte sedimentation rate within normal limits.
5-The results of tuberculin testing of cases show, 97 (48.5%) gave negative reaction with the diameter of induration < 10 mm and 103 (51.5%) gave positive reaction with the diameter of induration > 10 mm .
6-The X-ray examination showed few calcified foci, which have no signifigance in our study.
7- histopathological examination of serial sections from the removed tonsils, two tonsils out of 200 proved to contain tuberculous foci (1 %).
8-Bacteriological examination by direct smears and culture media were performed and two positive histopathological cases showed the Mycobacterium tuberculosis.
Consequently, involvement of the tonsil by tuberculosis may be under-diagnosed because it may not produce obvious symptoms or physical signs .it is important to consider tuberculosis in the differential diagnosis of tonsillar lesions and take biopsy specimens for histopathological and bacteriological studies, especially in countries where tuberculosis is seen frequently.
The diagnosis of tonsillar tuberculosis is based on the charac¬teristic histopathological granuloma. Positive findings of AFB from tissue will strongly confirm the diagnosis. Granulomatous inflammation can be found in fungal infection, syphilis, Wegener’s granulomatosis and others, although they are less common than tuberculosis.
In conclusion, the primary tuberculosis of the tonsil is exist, though its incidence is low, and its presence is not related to existence of other forms of tuberculosis like pulmonary or intestinal tuberculosis. its presence is observed only in children in contact with patients suffering from pulmonary tuberculosis .
Strongly positive tuberculin test as well as contact with tuberculous patients is indicator for possible presence of tonsillar tuberculosis where for microbiological and histopathological examination of removed tonsils is highly recommended.
